Systemic lupus erythematosus (SLE) is a clinically heterogeneous disease which is autoimmune in origin and characterized by the presence of auto antibodies directed against nuclear antigens. A 15-year-old female presented with a two-month history of progressive blurred vision in both eyes. The patient was on hydroxychloroquine 400 mg per day for eight months for the treatment of SLE. Eight months prior to presenting, the patient was found to have systemic lupus erythematosus and admitted in view of acute flares for three times. Fundus examination showed bilateral absent reflexes with retinal pigmentary changes. Hydroxy-chloroquine and chloroquine are licensed for the treatment of SLE. Hydroxychloroquine has been associated with many ocular toxic effects. The risk of retinal toxicity secondary to hydroxychloroquine can be divided into dose-related and patient-related factors. Out of these factors, daily dosage is thought to be the most important factor in the development of hydroxychloroquine retinopathy. Our patient developed severe retinal toxicity despite being on the recommended daily dose (400 mg per day). There are some evidences that termination of treatment in cases of early retinopathy may result in reversal of toxic damage and functional visual loss, moderate to severe retinopathy is permanent. This describes the consequence of screening to detect early signs of toxicity and prevent more serious retinal damage. This case places of interest the need to monitor signs of retinal toxicity in patients on hydroxychloroquine. And close alliance between the prescribing dermatologist and the ophthalmologist will help kind treatment for patients with drug induced ocular toxicity and avert permanent eye damage....
